From Data to Direction: What SNF Leaders Need to Know About VBP, QRP, NHSN and PBJ

Insights from Colleen Toebe, MSN, CWCN, RAC-MTA, RAC-MT, DNS MT – Vice President of Clinical Services
Skilled nursing facility leaders are managing more reports than ever before, and each one tells part of the facility’s quality, staffing, clinical, operational and financial story. VBP, QRP, NHSN and PBJ should not be viewed as separate reporting tasks completed by different departments. They need to be managed as one connected quality system. Leaders need to understand what each report measures, where the data comes from, how accurate the data is, and how the outcomes impact care, reimbursement, public reporting, survey readiness and overall facility performance. As noted in Pathway Health’s SNF Administrator Checklist, leaders should verify report access, monitor trends, validate data before deadlines and ensure findings are acted on through QAPI.
For leaders, this work matters because these reports are not just “numbers.” They reflect the facility’s systems in action. A readmission trend may point to gaps in change-of-condition management, discharge planning, physician communication or infection prevention. A staffing trend may help explain falls, call light response concerns, wounds, behaviors or hospital transfers. MDS and QRP coding concerns may identify documentation gaps, inaccurate quality measure results or missed opportunities to accurately reflect resident needs and outcomes. When leaders understand the reports, they are better prepared to ask the right questions, support the team, prioritize resources and prevent small issues from becoming larger regulatory, operational or financial concerns.
VBP: Value-Based Purchasing
Leaders need to understand that VBP has a direct connection to quality outcomes and financial performance. Current and upcoming VBP measures include readmissions, SNF healthcare-associated infections requiring hospitalization, discharge to community, long-stay hospitalizations, nursing staff turnover, total nurse staffing, discharge function and long-stay falls with major injury.
Key areas for leadership review include:
- Pulling and reviewing the latest SNF VBP Confidential Feedback Report from iQIES.
- Comparing current performance to prior quarters, baseline periods and CMS benchmarks when available.
- Identifying measures that are below target, declining or at risk for financial impact.
- Reviewing hospital transfers and readmissions by diagnosis, unit, shift, provider, hospital and root cause.
- Reviewing SNF HAI hospitalizations with the infection preventionist and medical director.
- Reviewing long-stay falls with major injury, including incident documentation, hospital records and MDS coding.
- Adding underperforming VBP measures to QAPI with an action plan, owner, due date and follow-up date.
QRP: Quality Reporting Program
QRP requires strong MDS accuracy, timely submissions and documentation that supports the resident’s condition, care needs and outcomes. Leaders should not assume that accepted submissions mean the data is complete or accurate. The facility must be able to support what was coded and submitted.
Key areas for leadership review include:
- Reviewing QRP reports in iQIES, including provider preview, review-and-correct and facility-level quality measure reports.
- Confirming required MDS assessments were accepted and investigating late, rejected, modified or corrected assessments.
- Auditing high-risk QRP MDS sections, including Section GG, Section J, Section M, drug regimen review, discharge items, transfer of health information and vaccination items.
- Comparing MDS coding to nursing notes, therapy notes, hospital records, physician orders, care plans and discharge summaries.
- Reviewing claims-based measure trends for hospitalization, discharge to community and potentially preventable outcomes.
- Reporting QRP risks, coding trends and missed documentation opportunities to QAPI monthly.
NHSN: Infection Prevention Reporting
NHSN reporting is more than a required submission. It helps leaders understand infection prevention risks, respiratory illness trends, vaccination reporting, outbreak activity and infection-related hospital transfers. These outcomes can also affect VBP, QRP, survey readiness and resident safety.
Key areas for leadership review include:
- Confirming NHSN submissions are completed by the infection preventionist or trained backup user.
- Reconciling NHSN reporting with infection line lists, laboratory reports, hospitalization logs, vaccine records and employee health records.
- Reviewing resident respiratory illness trends, including COVID-19, influenza and RSV during respiratory season.
- Reviewing resident and health care personnel vaccination reporting and identifying documentation gaps.
- Reviewing outbreaks, transmission-based precautions, enhanced barrier precautions and infection-related hospital transfers.
- Adding infection prevention trends that affect VBP or QRP to QAPI for follow-up.
PBJ: Payroll-Based Journal Staffing Data
PBJ data tells an important staffing story. Leaders need to understand how staffing levels, RN coverage, weekend staffing, agency use, turnover and vacant positions may impact both public reporting and resident outcomes. Waiting until the end of the quarter to review PBJ data creates unnecessary risk.
Key areas for leadership review include:
- Reviewing PBJ data weekly instead of waiting until the end of the quarter.
- Comparing PBJ data to payroll, time clock records, schedules, agency invoices and census.
- Confirming all direct care staff, agency staff and contract staff are captured correctly.
- Reviewing job codes for DON, RN, LPN, CNA, medication aide, nurse aide in training and other direct care categories.
- Reviewing daily RN coverage, total nursing hours per resident day, weekend staffing, call-ins and staffing gaps.
- Monitoring agency use, overtime, vacant positions, staff turnover and tenure trends.
- Discussing PBJ staffing trends in QAPI and connecting staffing to falls, wounds, infections, call lights, behaviors and hospital transfers.
QAPI: Where Reports Become Action
The most important step is moving report review into QAPI. Reports should not sit in a binder or electronic folder without follow-up. Leaders need to ensure that trends are discussed, risks are prioritized and action plans are developed with accountability.
Key areas for leadership follow-up include:
- Reviewing VBP, QRP, NHSN and PBJ trends at least monthly.
- Assigning corrective action plans with a responsible person, due date and follow-up date.
- Tracking root cause, action steps and measurable outcomes.
- Reporting findings to QAPI and the governing body as appropriate.
- Maintaining evidence of report review, validation, correction requests, action plans, submission confirmations and follow-up.
Three Key Leadership Strategies for Success
- Build one reporting calendar and one leadership rhythm.
Assign clear owners for VBP, QRP, PBJ, NHSN, MDS accuracy and QAPI follow-up. Review access, deadlines, reports, validation windows and correction opportunities at least monthly, with focused quarterly review before submissions. - Validate the data before it becomes the facility’s story.
Compare reports to source documentation, including MDS records, clinical notes, therapy documentation, payroll, census, infection logs, hospital records and submission confirmations. Do not assume the data is accurate because it was submitted. - Move every risk into QAPI with action and accountability.
Every underperforming measure, missed documentation opportunity, staffing trend or infection prevention concern should have an owner, root cause, action step, due date and follow-up result. The checklist reinforces that report findings should be converted into measurable action with evidence of completion.
Strong report management is strong facility leadership. When SNF leaders understand VBP, QRP, NHSN and PBJ, they can see how clinical care, staffing, documentation, infection prevention, reimbursement and quality outcomes are connected. These reports provide an opportunity to identify risk early, support teams more effectively, strengthen QAPI, improve resident outcomes and tell the facility’s quality story with confidence.
