NEW EPISODE: Assisted Living Individualized Service Plans — Tips and Tools

Individualized Service Plans (ISPs) play an important role in assisted living. More than just a clinical tool, ISPs are used to engage the whole team involved starting at the sales process and even including family. Taking it a step further, we can see where there are gaps and use them as a tool to provide education and ongoing training to staff.
ISPs are core to everything that is done in an assisted living community, serving as the core tool to determine resource needs, staffing needs as well as specific knowledge needed to provide great care to those you serve.
Consider the following to strengthen your organization:
- ISPs encourage teamwork. Not only will ISPs improve the lives of those you serve, but they also help to ensure collaboration and involvement of the entire team— and that means that your residents are in great hands. When an entire team is aware of the unique needs of every individual, there’s a much better chance that they will get the care and support they need every day, especially when they constantly involve the families and other health care professionals in their daily routines.
- ISPs encourage growth and improvement. ISPs help team members’ growth through monitoring residents, facilitating education, and identifying growth opportunities within assisted living communities. By setting clear expectations, maximizing electronic health records (EHRs), and leveraging available tools and resources, ISPs are designed to enhance each resident’s treatment plan since they’re always being recorded and improved upon.
- More ways to learn about ISPs. You can’t learn everything about ISPs just from doing research and looking at different assisted living communities — but you can use resources available at your fingertips. Try tapping into internal company resources, regional teams, local associations, state Departments of Health, and trade organizations for additional support in ISP development and implementation.
