Section GG: Where Functional Accuracy, Quality Outcomes, and Reimbursement Meet

Section GG is much more than another section of the MDS. It tells the resident’s functional story — how the individual performs with self-care and mobility, what progress is being made, and what support is truly needed to achieve the best possible outcome. For NHAs, DONs, and MDS Coordinators, Section GG should be viewed as a leadership priority, not simply an MDS coding task. CMS continues to emphasize accurate resident assessment through the RAI process, and the MDS 3.0 RAI Manual remains the primary source for Section GG coding direction and expectations.

The most common pitfalls we see are not always related to lack of knowledge — they are often related to gaps in process. Section GG accuracy can break down when nursing, therapy, and MDS teams are not aligned; when documentation does not clearly support the resident’s usual performance; when staff are unsure what to capture; or when documentation occurs after the fact instead of during the assessment window. Section GG data impacts care planning, therapy needs, PDPM reimbursement, SNF Quality Reporting Program measures, public reporting, and audit risk, making strong interdisciplinary capture essential.

Leaders should ask themselves a simple question: “Does our documentation tell the same functional story across nursing, therapy, MDS, and the care plan?” If the answer is no, there is opportunity. A strong Section GG process starts at admission, continues through the assessment period, and is reinforced through daily communication. Staff closest to the resident must understand that what they observe during transfers, toileting, eating, walking, bed mobility, dressing, and other daily routines matters. MDS Coordinators should not have to “hunt” for the resident’s functional status; the system should be designed so the data is captured timely, consistently, and with enough detail to support the final coding decision.

Section GG accuracy is not about chasing a score — it is about capturing the resident’s true functional ability and using that information to drive better care, stronger outcomes, and a more confident leadership process. When leaders build Section GG into daily operations, provide education to frontline staff, and review documentation before the MDS is locked, the organization is better positioned to support residents, strengthen reimbursement integrity, reduce audit risk, and improve quality outcomes.

Leadership Key Strategies

  • Build Section GG into the daily IDT rhythm. Review new admissions, functional changes, therapy input, nursing observations, and documentation gaps during stand-up, clinical meeting, or Medicare meeting so concerns are addressed before the assessment is completed.
  • Educate frontline staff on what matters. CNAs, nurses, therapy staff, and unit leaders need simple, practical examples of what to document, when to document, and why their observations directly impact care planning, reimbursement, quality measures, and compliance.
  • Audit before the MDS is locked. Use a quick pre-submission review to compare nursing documentation, therapy notes, progress notes, diagnoses, resident interviews, and care plan needs to ensure Section GG coding is supported and consistent.

How Pathway Health Can Assist

Pathway Health supports post-acute care providers with practical, operations-focused MDS and reimbursement solutions designed to strengthen accuracy, compliance, team confidence, and financial outcomes. Our team can assist with:

  • Section GG-Focused Review to evaluate current documentation, coding accuracy, interdisciplinary communication, and opportunities for improved functional data capture.
  • MDS Baseline Assessment to review the overall MDS process, identify risk areas, validate reimbursement drivers, and support quality and compliance alignment.
  • MDS Process Education for MDS Coordinators, DONs, nursing leadership, therapy partners, and frontline staff to promote accurate and timely documentation.
  • MDS Mentoring to support new or developing MDS Coordinators through real-time guidance, process coaching, and practical leadership support.
  • MDS Interim Services to provide experienced MDS support during vacancies, transitions, leave coverage, or periods of increased operational need.
  • Customized reimbursement, clinical documentation, QAPI, PDPM, audit preparation, and operational consulting support.
