Survey Readiness Is a Team Sport: Preparing Your Nursing Home or Assisted Living Community Before Surveyors Arrive

Insights from Colleen Toebe, MSN, CWCN, RAC-MTA, RAC-MT, DNS MT – Vice President of Clinical Services
Whether you lead a skilled nursing facility or an assisted living community, survey readiness should never begin when surveyors walk through the door. The strongest organizations I visit across the country have one thing in common: they treat readiness as part of daily operations, not as a once-a-year event. Administrators, Executive Directors, Directors of Nursing, Assisted Living Clinical Directors, department leaders, and frontline caregivers all play a role in creating an environment where quality, compliance, and resident-centered care are visible every day.
A team approach is essential because survey readiness touches every corner of the building. Clinical documentation, medication management, infection prevention, resident rights, staff competency, environmental safety, dining services, life safety, and quality improvement are all connected. When one department is working in isolation, gaps are easier to miss. When leaders round together, review trends together, and coach staff together, the organization becomes more consistent and more confident. Surveyors will not only review records; they will observe care, interview staff, speak with residents and families, and look for whether your systems are truly working.
Upskilling your team is one of the most effective ways to reduce survey risk. Staff need more than annual education or a binder of policies. They need practical, ongoing coaching that helps them understand the “why” behind expectations and how to apply those expectations in real situations. Can your team explain what to do after a fall? Do they know how to report a concern? Can they speak to a resident’s care plan, preferences, risks, and interventions? In both nursing homes and assisted living communities, confidence comes from repetition, competency validation, and leaders who are present on the units and in the neighborhoods.
Leaders should also make full use of state and federal resources that apply to their setting. For nursing homes, CMS guidance, the State Operations Manual, Quality Measure reports, Care Compare data, infection control resources, and state survey agency updates can help identify areas of focus. Assisted living leaders should stay closely connected to state-specific regulations, licensing updates, survey trends, and provider association resources. Because assisted living regulations vary significantly by state, it is especially important to know what your state expects and how those expectations are being interpreted during surveys.
Finally, know the common citations in your state and region. Survey patterns are not random. Most states and regions have recurring focus areas, such as infection prevention, falls, medication storage and administration, resident rights, abuse reporting, care planning, documentation, staffing competency, food safety, and environmental concerns. Reviewing your own history, complaint trends, incident reports, quality indicators, and regional citation patterns can help you prioritize preparation. The goal is not to “prepare for survey week.” The goal is to build a culture where your team is ready to show the good work they do every day.

Key Leadership Strategies
- Build survey readiness into daily leadership routines. Use interdisciplinary rounding, department huddles, and focused audits to identify and correct issues in real time.
- Upskill and validate staff competency. Provide practical education, observe performance, ask staff common survey questions, and follow up with coaching when gaps are found.
- Use data and resources to guide priorities. Review state and federal guidance, quality measures, complaint trends, incident patterns, and common citations in your state and region.
- Create shared accountability across departments. Survey readiness is not only a nursing responsibility. Environmental services, dietary, activities, maintenance, social services, administration, and clinical leadership all need to understand their role.

Complimentary Survey Preparation Tool: Leadership Fall Prevention Program Checklist
Falls continue to be a top citation area across care settings, including skilled nursing facilities and assisted living communities. A strong fall prevention program requires more than identifying residents at risk. Leaders need to evaluate whether systems are working, interventions are individualized, staff are following through, and post-fall reviews are leading to meaningful change.
Pathway Health’s Leadership Fall Prevention Program Checklist is a complimentary tool designed to help leaders identify potential areas for improvement as part of their survey preparation process. Use it during leadership rounding, QAPI review, clinical risk meetings, or mock survey preparation to evaluate your current practices and strengthen your fall prevention systems before surveyors arrive.
