MDS Accuracy Under the Microscope: What SNF Leaders Need to Know Now

Insights from Scott Heichel, RN, RAC-MT, RAC-CTA, DNS-CT, IPCO, QCP, ICC – Director of Reimbursement and Education, Pathway Health
MDS accuracy has always mattered, but the level of scrutiny surrounding assessment data continues to increase. For skilled nursing leaders, the MDS is no longer simply a regulatory assessment or reimbursement tool; it is one of the primary data sources CMS uses to evaluate quality, determine payment implications, inform Five-Star ratings, support SNF Quality Reporting Program requirements, and measure performance under value-based purchasing. When the MDS does not accurately reflect the resident’s condition, care needs, diagnoses, functional status, medication use, and clinical interventions, the impact can extend well beyond one assessment. It can affect publicly reported outcomes, survey focus, payment accuracy, compliance exposure, and leadership’s ability to understand the true performance of the organization.
The new hybrid long-stay antipsychotic quality measure reinforces how CMS is expanding its view of facility performance. By incorporating both MDS data and Medicare/Medicaid claims information, CMS is signaling that quality measure oversight will increasingly depend on whether the full clinical, billing, pharmacy, and diagnostic record aligns. For SNF leaders, this means antipsychotic medication use, exclusion diagnoses, physician documentation, gradual dose reduction efforts, behavioral interventions, pharmacy recommendations, and MDS coding must all tell the same accurate story. Facilities can no longer rely on isolated department-level review. The NHA, DON, MDS Coordinator, therapy leadership, billing team, medical providers, and consultant pharmacist all have a role in ensuring the record supports the care delivered and the data submitted.
Preparing for CMS quality audits requires a proactive and disciplined approach. Leaders should not wait for a validation review, survey concern, quality measure spike, or payment issue to discover inconsistencies. Routine internal MDS audits, focused documentation reviews, and interdisciplinary quality measure analysis should be part of ongoing operational oversight. This includes reviewing whether assessments are timely, diagnoses are supported, Section GG coding is accurate, care plans match resident needs, quality measure triggers are understood, and claims accurately reflect skilled services provided. Strong MDS systems help facilities identify risk early, support appropriate reimbursement, improve quality outcomes, and create confidence that submitted data is accurate, defensible, and aligned with CMS expectations.
Leadership Strategies for SNF Readiness
- Make MDS accuracy a leadership-level priority.
Include MDS accuracy, quality measures, SNF QRP, SNF VBP, Five-Star outcomes, and reimbursement trends in routine clinical, QAPI, and operational meetings. - Conduct routine MDS and documentation audits.
Review a defined sample of records each month, with focus on high-risk areas such as diagnoses, Section GG, psychotropic medications, skilled coverage, PDPM drivers, quality measure triggers, and care plan alignment. - Break down departmental silos.
Ensure nursing, MDS, therapy, billing, social services, medical providers, pharmacy, and leadership are communicating consistently so documentation, coding, claims, and quality outcomes are aligned. - Prepare specifically for the hybrid antipsychotic measure.
Validate exclusion diagnoses, review antipsychotic utilization, assess behavioral documentation, confirm gradual dose reduction and non-pharmacological intervention documentation when appropriate, and compare MDS coding with pharmacy and claims data.
How Pathway Can Assist

Review of claims, skilled documentation, PDPM drivers, supporting clinical evidence, and billing practices to support accuracy and compliance. - Revenue Optimization Review
Evaluation of opportunities to support appropriate reimbursement through accurate coding, complete documentation, and alignment between resident acuity and the services provided. - Hybrid Antipsychotic Measure Readiness Review
Targeted analysis of antipsychotic use, supporting diagnoses, pharmacy documentation, behavioral interventions, MDS coding, and claims alignment. - MDS Training and Mentoring
Education and hands-on support for MDS Coordinators, DONs, IDT members, and facility leaders to strengthen coding accuracy, documentation practices, and regulatory understanding. - SNF QRP, SNF VBP, and Five-Star Support
Review of quality performance trends, risk areas, data accuracy, and leadership action plans to support ongoing quality improvement. - Interim MDS and Reimbursement Support
Temporary or project-based support to assist facilities during staffing transitions, backlog situations, audit preparation, or focused performance improvement efforts.
With the growing connection between MDS data, quality outcomes, claims, and reimbursement, accuracy must be built into the daily operating rhythm of the facility. Pathway can help leaders assess current systems, identify vulnerabilities, educate teams, and implement practical strategies that support compliance, quality performance, and financial integrity.
